Photo Release Waiver
I understand that participants, including myself and my child, in Code Red Athletics may be included in photographs and/or videos taken by spectators, parents, coaches, staff members, and/or by professional photographers.
Appropriately, I give Code Red Athletics and their designees, the right to use the Photos as well as my name, my child’s name (first names only), and likeness in any manner and without any compensation to me or my child. I waive any right to inspect or approve the final version of the Photos and/or any accompanying written materials.
Examples of Code Red Athletics use of the Photos include social media, e-mail marketing, printed marketing materials, the Code Red Athletics website, signage, and on other materials used to promote Code Red Athletics.
I am a parent/legal guardian to my child, who is a participant, and I agree to grant and release explained above to me and my child to all of its terms.
I have read this photo release and fully understand its terms. I also understand that I have given up certain rights by signing this release, as explained above, and I voluntary agree to do so.
Athletic Waiver
By signing this waiver form, I grant permission for the child named above to participate in and engage in the Code Red Athletics events. My child is physically and mentally able to participate in these activities, unless I have already discussed it with one of the coaches. I acknowledge that there are certain risks involved in said activities, and have discussed them with my child if necessary.
I release Code Red Athletics, its affiliates, Community Fieldhouse, volunteers, and employees of all responsibilities for any injuries, to body or property, which may occur to my child during the course of these activities. In the event of an emergency in which I, or the alternate contact, cannot be reached, I authorize the adult leaders to make medical decisions for my child, and to administer first aid if deemed necessary.
I further agree to indemnify and hold harmless Code Red Athletics and its affiliates, volunteers, and employees of any and all claims arising from the participation of my child in activities or as a result of injury or illness of my child or mine during such activities. I represent that I am the participant, or the parent/guardian of the child listed above who is under 18 years of age. I have read the Permission/Waiver Form and I am fully aware of its contents. With this signature, I give permission for the child named above to participate fully in the activities of Code Red Athletics.
